Brain Morbidity in Treatment-Naive Alcoholics
Most studies on the impact of alcohol dependence on the brain have examined individuals in treatment, or shortly thereafter. Such samples represent only about a quarter of alcohol dependent individuals in the general population.
In the previous five years of work, we studied 21-50 year old actively drinking treatment-naive alcohol dependent individuals and age and gender comparable controls, and compared them to our other ongoing study of 35-55 year old long-term abstinent treated alcohol dependent individuals and their age and gender comparable controls. We showed that with regard to alcohol intake in the initial period after starting abusive drinking, treatment-naive alcohol dependent individuals were a subpopulation with lower initial heavy alcohol consumption than abstinent treated and not simply observed earlier in the progression of the disease. The current study will continue to examine the same issues in an older sample of 35-60 year old treatment-naive alcohol dependent individuals, half with and half without concomitant drug abuse/dependence.
Long-term Abstinent Alcoholics: Clinical Issues and Central Nervous System (CNS) Disinhibition
Extends our work in studying long-term abstinent alcoholics with comparison samples of short-term abstinent alcoholics, and both short- and long-term abstinent alcoholics with concomitant drug and alcohol dependence.
Our preliminary/prior data showed clearly that LTAAs were very deviant prone, poorly socialized, made decisions that were strongly governed by immediate rewards, and evidenced reduced amygdala volumes, all characteristics associated with antisocial behavior, yet engaged minimally in such behavior. A focus of the work proposed here is to determine whether the neurobiological disinhibitory underpinnings of externalizing illness have changed with abstinence, or whether the predisposition is unchanged, but kept under wraps by higher cognitive functions. In addition, this would allow us to replicate, with greater sensitivity, our prior work in long-term abstinence, recruiting both short- and long-term alcoholics with comorbid drug dependence, so that the generalizability of our potential findings to the U.S. population would be (greatly) improved. Moreover, we believe that the findings from alcohol-dependent individuals with versus without comorbid drug dependence will differ with regard to the function of disinhibitory brain systems, comorbid psychiatric disturbance, and cognitive recovery. Our purpose is not to look at specific drug effects, but to test the hypothesis that individuals with comorbid drug dependence are sicker; i.e., having greater disability and lesser recovery than individuals with alcohol dependence alone.
